Doesn't that "notrust" make his ntpd ignore your server, as it means
"must use crypto" in ntp 4.2 ?  Maybe this combination causes the
per-second requests you see ...  He should try changing it to nopeer.

Would be worth checking "ntpdc -c iostats -c sysstats", before
changing the restrict.

Unreachable, yet you receive his requests: implies a problem at his end.
