I'm assuming ntpd tries to synchronize the Windows time once every few 
minutes, after a few polling intervals. If the determined time 
difference then exceeds 128 ms (which is probably the case on this 
machine) then the system time is stepped, all filter entries are 
discarded, and "reach" is set back to 0.

There should be related log entries in the application event log, if you 
have set the log level high enough.

Maybe you should add

logconfig =all

to your ntp.conf file if this isn't the case. Please not =all needs to 
be typed as a single word.
