Dr. Mills approved Dave Hart's pending set of changes to ntp_proto.c, including
a couple of enhancements and fixes for:

[Bug 988] [Bug 1921] [Bug 1922] [Bug 1923] [Bug 1924]

These are ready for ntp-dev in the repo referenced below.  After some ntp-dev
exposure, the bug fixes (but not the scope expansion for "tos orphanwait")
should be backported to 4.2.6, if there is to be another 4.2.6 release.
