Yes, one bad server out of four is why four servers are configured. However, this server is consistently way off and for practical purposes when it's supplied by the pool it's as though there were just three servers. When I notice that the pool gave it to my systems, I just restart NTP to get a new set of servers.
I suspect that this server was configured as a high-bandwidth one and often makes to the top of the pool response to a DNS query. I really think that high-bandwidth servers should either reach the cut off threshold faster or have a higher cut-off threshold proportional to the stated bandwidth. IMO, it shouldn't more than a day to cut off a bad or a poorly configured server.
