Thanks to all the useful suggestions on here it's all working pretty well 
now on Win 7 x64.

Reducing the DPC latency by setting the Win 7 power management profile 
made the most difference.

Using ntp-4.2.7p450-win-x86-bin-djt tweaked with imagecfg to run on the 
last CPU.

Running with the -M switch to force nptd to use the Windows clock directly.

Experiments without the -M switch and playing media were inconclusive.

ntp.conf...

logconfig =all

driftfile "C:\Tools (x86)\NTP\etc\ntp.drift"

enable stats
statsdir "C:\Tools (x86)\NTP\etc\"
statistics loopstats
filegen loopstats file loopstats type day enable

restrict source nomodify
restrict 127.0.0.1
restrict -6 ::1

pool uk.pool.ntp.org iburst minpoll 6 maxpoll 6

Still a bit unsure about the restrict statements.  Need to read some more.

precision seems to stick at -10 (0.977ms) but offset now stays within 15ms.
