The problem seems to be in the signalling between the iocompletion thread and
the main thread (GetReceivedBuffers()).

I just conducted a test where I commented out the call to
SetEvent(WaitableIoEventHandle) in OnSocketRecv() and then changed the call to
WaitForMultipleObjects() to have a time out of 500 (milliseconds) instead of
INFINITE.

That worked and I am no longer seeing Access Violation failures.

So the problem is with the event handles. I'll try a number of things to see if
this will solve the problem.
