[ntp:hackers] -dev and bug 657 under Windows

I've just cloned a fresh ntp-dev repo and tried to build under W2k using
VC6. There are still a couple of warnings due to unresolved symbols at
link time, so I'll have to add a few C source files to the project.

I've started with ntpd-opts.c which returns a couple of warning due to a
couple of undefined symbols (USHRT_MAX, SHRT_MAX). Those are in limits.h
 which must be included under Windows.

Since there's currently no way to run autogen under Windows, I've edited
ntpd-opts for now. After I've found all required modifications we can
see where we'll have to modify the autogen input files such that the
generated files are OK.

Of course I can't run ntpd before I can build it under Windows, so I
can't tell right now whether those bugs are resolved, or not.
